• A photograph of you that’s at least 100 pixels wide and no more than 120 pixels high (If it’s larger, I’ll crop and/or scale it as needed)
• Your city and state, province or country
• The name and URL of the site you’d like me to link to (no adult or gambling sites, sites penalized by Google, or sites linking to such sites)
